Ministers Reporting Information

This year ALL Reporting forms for all licensed Ministers (for those not filing a Senior Pastors Report) are available here for you to download. Please complete those pertaining to your ministerial role and return to the district office at the designated time. If you have any questions feel free to contact the district office (513) 777-5600.

Please email all completed forms to ccox@swonaz.org or mail to the SWO District Office, 7943 Tylersville Rd. West Chester, OH 45069.

Also, we will need to know if you are issuing a local license to any member of your congregation this year, if so, please email ccox@swonaz.org. If you are aware of any individuals that have had their local license for one year and have met the recommended course of study up to this point, they will need to attend the Assessment Orientation on March 6, 2010, 10:00 AM at the district office. Please refer to the Team Update for additional information.

Important Dates

Assessment Orientation and Material Distribution – March 6, 2010, 10:00 AM, District Office

Assessment Conference – April 17, 2010, 9:00 AM (Place to be determined depending on number of candidates)

Ministerial Credentials Day – Saturday, May 1, 2010
